Incoming Nursery, Kinder, Grade 1, and Grades 2-6 New Pupils Transferees

Admission Requirements

Nursery and Kinder

1. Valid ID of Guardian (Photocopy)
2. One 2×2 Picture of Pupil
3. Two (2) PSA Birth Certificate (Photocopy)
4. Report Card/SF9 (Original and Photocopy)

Note:  For incoming kindergarten, the student must be 5 years old on or before October 31 of the current year.

For Incoming Grade 1

1. Valid ID of Guardian (Photocopy)
2. One 2×2 Picture of Pupil
3. Report Card/SF9 (Original and Photocopy)
4. Two (2) PSA Birth Certificate (Photocopy)
5. Form 137 (Original and Photocopy)
6. Good Moral Certificate
7. Certificate of Kindergarten Completion

Transferees

1. Valid ID of Guardian (Photocopy)
2. One 2×2 Picture of Pupil
3. Report Card/SF9 (Original and Photocopy)
4. Two (2) PSA Birth Certificate (Photocopy)
5. Form 137 (Original and Photocopy)
6. Good Moral Certificate
7. Baptismal Certificate (For Grade 4 Roman Catholic students ONLY)

Enrollment Procedures

For New Pupils/Transferees and Returning Students

1. Register at https://portal.ubaguio.edu.
2. Click “Apply as a New Student”, complete all the needed information and submit the registration.
3.    You will receive an email containing your APPLICANT ID NUMBER that will be used to pay the Reservation fee of Php 2,000 and a google form link https://forms.gle/DqWEN7cUhR8k8mQSAforms.gle.
   
   NOTE: Reservation fee is non-refundable.
   
4. Read and follow all instructions posted in the google form while filling out all required data.
5. After accomplishing the Reservation Form link, wait for the schedule of enrollment for New and Transferees students to be posted on the UBLES Facebook Page (https://www.facebook.com/UBLES1951)
6. Proceed to UBLES on the scheduled date for the face-to-face enrollment carrying the complete requirements. (Students will ONLY be enrolled when requirements are complete)
