Admission Requirements
**Original and photocopy of the documents should be placed in a long folder with complete label
Incoming Grade 7
- Original and 2 Photocopies of Report Card Back-to-Back (School Form 9)
- 3 Photocopies of PSA Birth Certificate
- 3 Copies of recent 2 × 2 ID Picture for Student with nametag and white background
- 1 Copy of recent 2 × 2 ID Picture for Parent/Legal Guardian with white background
- Latest Income Tax Return / Certificate of Tax Exemption from BIR
- ESC Application Form and ESC Grantee Enrolment Contract (Will be given to the applicants once the requirements are completed)
Transferees
- Original and 1 Photocopy of Report Card Back-to-Back (School Form 9)
- 2 Photocopies of PSA Birth Certificate
- 1 Copy of recent 2 × 2 ID Picture for Student with nametag and white background
- 1 Copy of recent 2 × 2 ID Picture for Parent/Legal Guardian with white background
- ESC Certificate (For Grade 8-10 who are ESC Grantees from previous schools)
Returnees/Balik-Aral (For 1 & 2, whichever document is applicable)
- Original and 1 Photocopy of Report Card of last School Attended Back-to-Back (School Form 9)
- PEPT Certificate of Rating
- 2 Photocopies of PSA Birth Certificate
- 1 Copy of recent 2 × 2 ID Picture for Student with nametag and white background
- 1 Copy of recent 2 × 2 ID Picture for Parent/Legal Guardian with white background
Foreign Students from Schools Outside the Philippines
- Special Study Permit issued by the Bureau of Immigration
- Five (5) Copies of duly accomplished and signed Original Personal History Statement with attached 2X2 photos of Applicant
- Report Card from the School/ Institution the Applicant last attended
- Permanent Transcript of Records (Original and Photocopy) duly authenticated by the Philippine Embassy or Consulate based in the Applicant’s Country
- A duly notarized Affidavit of Support accompanied by a Bank Statement duly authenticated by the Philippine Embassy or Consulate based in the Applicant’s Country
- Two (2) photocopies of Certificate of Live Birth and/ or Passport Pages where name, photo, birthdate, and birthplace appear and duly authenticated by the Philippine Embassy or Consulate based in the Applicant’s Country
- PEPT Certificate of Rating for learners who did not complete the Academic Year abroad as long as THREE Grading Periods have been completed, and for learners who need Grade Level Standards Assessment
- Admission Fee of $ 400.00 (One-time payment only)

Enrollment Procedures
- Register at the UB Student Portal
- Click “Apply as a New Student”. Provide all the needed information and submit the registration.
- Prepare scanned copies of the documents for admission.
- Pay the reservation/registration fee of Php 1000.00 to any of the UB accredited banks and
prepare a scanned copy of the acknowledgement receipt.
• The reservation fee is non-refundable but creditable to the school fees once the student is
enrolled.

- Receive an e-mail of your reservation approval, once the uploaded documents are accepted and verified.
- For Transferees: Grades shall be evaluated prior to approval.
- For Foreign students outside the Philippines: Submit admission documents to the Principal’s Office for evaluation and validation.

Enrollment Proper
Online Enrollment
- Accomplish the online enrollment Google Form. Attach the scanned copies of the enrollment requirements.
- Encode the grade, section, and subjects under the student’s program and curriculum.
- You will receive an email informing you about your ID number, procedure of accessing the UB portal and activating your UB email address.
- Pay the required down payment in any of the accredited banks or at the UB Cashier. PTA Fee (Php200.00) shall be paid to the enrollment adviser during the first week of classes.
- View your schedule on the Student Portal.
Onsite Enrollment
- Proceed to the enrollment area to secure your schedule. Bring the required documents for enrollment, placed in a long folder with proper label.
- Pay the required amount at the UB Cashier. PTA Fee (Php200.00) shall be collected by the PTA Officer / Enrollment Adviser at the enrollment area.
- Go back to the enrollment area for the printing of your class schedule.
- Process your ID at the MIS Office.
