Admission Requirements

**Original and photocopy of the documents should be placed in a long folder with complete label

Junior High (Grade 7 Transferees)

  1. School Form 9 (Report Card) (Original & Photocopy)
  2. PSA Birth Certificate (2 copies) (Photocopy)
  3. Recent 2×2 ID Picture of Student (w/ Name Tag & White Background) (2 copies)
  4. Recent 2×2 ID Picture of Parent/Guardian (2 copies)
  5. Income Tax Return/ Certificate of Tax Exemption from BIR (Grade 7 Only)
  6. ESC Certificate from previous ESC Participating School, (Transferees Only)
  7. Certificate of Good Moral Character (Original & Photocopy) for COA Program and referral purposes only
  8. Photocopy of Kindergarten Certificate of Completion (Non UBLES Graduate and Balik-Aral Learner covered by the K+12 Curriculum

Returnees/Balik-Aral (For 1 & 2, whichever document is applicable)

  1. School Form 9 of last School Attended (Report Card) (Original & Photocopy)
  2. PEPT Certificate of Rating
  3. Photocopy of PSA Birth Certificate (2 copies)
  4. Recent 2X2 ID Picture of Student (with Name Tag & White Background)
  5. Recent 2X2 ID Picture of Parent/ Guardian

Students from Non-DepEd Accredited Schools (Private schools which did not undergo government accreditations)

  1. PEPT/ PVT Certificate of Rating
  2. Photocopy of PSA Birth Certificate (2 copies) 
  3. Recent 2X2 ID Picture of Student (with Name Tag & White Background)
  4. Recent 2X2 ID Picture of Parent/ Guardian

Enrollment Procedures

  1. Register to https://portal.ubaguio.edu
  2. Click “Apply as a New Student”, completes all the needed information and submits the registration
  3. You will receive a confirmation email with the link for the submission of scanned copy of the admission requirements.
  4. Pay the reservation/registration fee of Php 1000.00 to any of the UB accredited banks and prepare a scanned copy of the acknowledgement receipt.•The reservation fee is non-refundable but creditable to the school fees once the student is enrolled.
  5. The student/parent/legal guardian (in the case of the Grade 7 – Grade 10 learner) scans the following documents for admission:
    • School Form 9 (Report Card) (Original & Photocopy)
    • PSA Birth Certificate (2 copies) (Photocopy)
    • Recent 2×2 ID Picture of Student (w/ Name Tag & White Background) (2 copies)
    • Recent 2×2 ID Picture of Parent/Guardian (2 copies)
    • Income Tax Return/ Certificate of Tax Exemption from BIR (Grade 7 Only)
    • ESC Certificate from previous ESC Participating School, (Transferees Only)
    • Certificate of Good Moral Character (Original & Photocopy) for COA Program and referral purposes only
    • Photocopy of Kindergarten Certificate of Completion (Non UBLES Graduate and Balik- Aral Learner covered by the K+12 Curriculum)
  6. The student/parent/legal guardian (in the case of the Grade 7 – Grade 10 learner) uploads the scanned documents through the link at the end of the form.
  7. The student/parent/legal guardian (in the case of the Grade 7 – Grade 10 learner) sends the original copy of the documents to:

Submit the Enrollment Requirements to the UB High School Principal’s Office. (Only folders with Complete Documents shall be accepted)
  1. For Transferees:  Grades shall be evaluated by the principal prior to approval.
  2. For Learners from Philippine Schools Abroad and Foreign Schools Abroad: Submit admission documents to the Admissions and Records Center for evaluation and validation.
  3. ARC Forward documents with the identified Grade Level to which the applicant will be placed to the corresponding Secretary (JHS/ SHS).
  4. Upon receipt of the scanned documents, the Principal evaluate and approve the application and registration of learners except those from Philippine Schools Abroad and Foreign Schools Abroad.
  5. Student, Parent/ Legal Guardian will receive an email of approval and the list of admitted students in the portal will be updated.
  6. Enrollment Adviser/School Secretary/Assistant Principal encode the Grade, Section and subjects based on the student’s program and curriculum.
  7. Receive an email with ID number, procedures on how to access the enrollment portal and how to activate UB email address.
  8. Pay the required down payment, Php200.00/family (JHS) and Php200.00/family (SHS) PTA fee in any of the accredited banks or at the UB Cashier.  (Separate transaction receipt for Down payment and PTA)
  9. Check the schedule on the Student Portal.
