Admission Requirements
For Grade 12 Students expected to graduate at the end of the current school year.
- Certified true copy of GRADE 11 report card (F-138/SF9) with complete and passing grades and GRADE 12 first semester report card (F-138/SF9) duly signed by the school principal.
- CERTIFICATE OF NO FAILED GRADES filled out by the school principal.
- DECLARATION, WAIVER, AND CONSENT FORM
- Photocopy of PSA Birth Certificate
Note:
Students born abroad should submit a photocopy of Philippine passport (BIO-PAGE) or a Philippine Recognition Certificate.
For Grade 12 students who graduated prior to the current school year and has not enrolled in any tertiary schools before:
- Original and Photocopy of GRADE 12 report card (F-138/SF9) duly signed by the school principal.
- Photocopy of PSA Birth Certificate.

Students born abroad should submit a photocopy of Philippine passport (BIO-PAGE) or a Philippine Recognition Certificate.
For Undergraduate Students – Non-formal Secondary Graduates
- Certified True Copy of the Original Diploma released by the DepEd Central Office
- Original Certificate of Rating for Passer of the Alternative Learning System (ALS)
- Photocopy of PSA Birth Certificate
- Note: Students born abroad should submit a photocopy of Philippine passport (Bio-page) or Philippine Recognition Certificate

Reservation for Incoming Freshmen
On-Site Reservation
- Prepare photocopy of all admission credentials
- Register on the UB Student Portal
- Click “Apply as New student, complete all the needed information, and click submit registration
- Fill-out the Declaration, Waiver, and Consent Agreement Form
- Submit the original and photocopy of admission requirements to the admission officer, once verified;
- Courses will be reserved based on the student’s program and curriculum.
- Pay the reservation fee of Php. 2,000.00 at the UB Cashier and present the receipt to the admission officer.

Note:
- To complete your reserved courses to enrolled status, submit the original F-138 (SF9) report card and other requirements that were not submitted during the reservation period.
Reminders:
- Reserved slots are valid until June 6, 2025.
- Visit the ARC Office as soon as all credentials are available.
- Unconfirmed slots will be given to those with complete enrollment credentials.
Online Reservation
- Register on the UB Student Portal
- Click “Apply as a New Student”, complete all the needed information
- Once complete, click submit the registration. You will receive an email regarding your applicant ID NUMBER that you will use for the reservation fee.
- Pay the Reservation fee of Php. 2,000.00 at any UB accredited banks.
(For more information on this, check the university’s Payment Procedures.) - Download and fill-out the Declaration, Waiver, and Consent Agreement Form; make sure all the details are provided and duly signed.
- Prepare and scan the admission requirements (every document in PDF format only)

- Fill out and provide necessary iformation and upload the scanned documents in the Pre-Registration Google Form

- Submit the Original F-138 (SF9) report card and other requirements that were not submitted during the reservation period.

Enrollment Procedure
- Proceed to your Senior High School Principal’s Office and present the Certificate of No Failing Grades
Note: This must be accomplished by the principal. - Fill out the Declaration Form
- Pay he reservation fee of Php 2,000.00 at the UB Cashier.
Note: Reservation fee is non-refundable but will be credited to the tuition fee once enrolled - Present all receipts and all accomplished documents to the admission officer
