Admission Requirements

• CERTIFICATE OF NO FAILING GRADES
• DECLARATION, WAIVER, AND CONSENT FORM

Enrollment Procedure

1. Proceed to Senior High School Principal’s Office and present the Certificate of No Failing Grades
   NOTE: This must be accomplished by the principa.
2. Fill out the Declaration Form
3. Pay the reservation fee of Php 2,000.00 at the UB Cashier
   • NOTE: Reservation fee of Php 2,000.00 is non-refundable, but will be credited to the tuition fee once enrolled.
4. Present all receipts and accomplished documents to the admission officer.
