Requirements for Application
- The applicant must be an incoming first-year student, male or female between 17-20 years old;
- The applicant must be a resident of the Cordillera Administrative Region;

- Furnish a clear copy of the following:
- Fully accomplished Application Form
- One (1) recent 2×2 ID picture
- Senior High School Report Card (Grade 12) photocopy
- Certificate of Good Moral Character photocopy
- Certificate of Registration photocopy
- PSA Birth Certificate photocopy
- Photo of your house (at least 3 pictures)
- Endorsement from the Chief of the Tribe (belonging to Indigenous People)
- Certificate of Tax Exemption from Bureau of Internal Revenue
- Kindly submit all requirements to your RESPECTIVE DEAN’S OFFICE.
