
The Office of Student Affairs (OSA) has officially opened applications for both Leadership and Media Awards for the incoming June 2025 graduation. The process is as follows:

- Fill out the form and attach all necessary documents in a long plastic folio and submit the portfolio to your Dean.
- Application Form
- Resume
- Certificate of Good Moral Character
- Original certificates and supporting documents
- For a single seminar with multiple topics, only one certificate shall be considered
- If the nominee has multiple functions in an activity, their functions will be counted as one
- Period of coverage of all activities, membership, officership, and awards shall be from the applicant’s first year in the university up to the final Saturday preceding the scheduled deliberation date.
- A narrative describing your leadership experience that is no longer than 1,000 words
- School publications must be submitted to the Linkages Office on or before March 31, 2025. Submissions received after this date will not be included in the total number of publications.
- The Dean will screen your portfolio and validate all documents before submitting them to the Office of Student Affairs.
Deadline for submission of application will be on or before May 3, 2025. The office shall not be entertaining any application after the deadline.
