Application for scholarships and discounts for the second semester (Siblings, Solo Parent, PWD only) will be open from January 20 to January 25, 2025. Please use this link for your application.
Guidelines for Scholarships and Discounts Application Process
- Submit Your Application
– After applying for any scholarships or discounts, be sure to click “Submit”.
– You will immediately receive an email from Google Forms with a copy of your complete responses. If you do not receive this email, your application was unsuccessful.
– If you need to make changes, you can edit your responses using the same application link. - Incomplete Applications
– Applications with missing requirements or incomplete information will not be processed.
– Failure to follow the instructions will result in disqualification. - Submission of Original Documents
After a successful online application, students must submit to the Student Accounts Office the following:
– Solo Parent Discount: Submit a photocopy of a valid Solo Parent ID, along with a photocopy of the student’s birth certificate. Present the original ID for validation.
– PWD Discount: Submit a photocopy of a valid PWD ID. Present the original ID for validation. - Deadlines
– Online Application: Submit by January 25, 2025
– Original Documents: Submit by January 27, 2025 - Important Notes
– Non-submission of required documents on time will result in disqualification, even if the online application was successful.
– Scholarships/discounts will be posted within the semester after the 1st grading exam.
Important Reminders
- 1. To access the Google Form, login using your UB Student email ([email protected]). Only applications submitted through this account will be considered. If you experience difficulty accessing the form, ensure that you have logged out of all other accounts on your device and sign in solely with your UB Student email.
- The application period ends on January 25, 2025. Late submissions will not be entertained, and the Google form link (bit.ly/3DYKuBe) will be deactivated after this date. No extensions will be granted.
- Upon successful submission, you will receive a confirmation email with a copy of your responses.
- Submit an Original Copy of the documents uploaded in the Google form (except for Sibling’s Discount).
- Students residing in Baguio City/La Trinidad, Benguet may personally submit to the Student Accounts Office.
- Non-Baguio/La Trinidad residents may send through courier (LBC, JRS, etc.) the original copy of the required documents to:
