Application Process
- Student must download the application form using this link:
https:forms.gle/ycM8HG5QKXYCtuon8 - Student must fill out the form and attach the following necessary documents in a long plastic folio:
a. Application form
b. Resume
c. Certificate of Good Moral Character
d. Original certificates and supporting documents
*For a single seminar with multiple topics, only one certificate shall be considered.
**If the nominee has multiple functions in an activity, his/her functions will be counted as one.
***Period coverage of all activities, memberships, officerships, and awards shall be from the applicant’s first year in the university up to the final Saturday preceding the scheduled deliberation date.
e. A narrative describing the student’s leadership experience that is no longer than 1,000 words.
Once the documents are complete, the student must then submit his/her portfolio to his/her respective Dean. - School publications must be submitted to the Linkages Office on or before September 30, 2024. Submissions received after this date will not be included in the total number of publications.
- The Dean screens the students’ portfolios and validates all documents before submitting them to the Office of Student Affairs (OSA).
The deadline for applications is December 7, 2024.
The deliberation period for academic and non-academic awardees will be on January 10, 2025.
